Three people fainted and another had a seizure after watching a gruesome arm amputation scene in new James Franco movie 127 Hours.
Viewers fled the cinema at the Toronto Film Festival while others refused to look at the screen when Franco cut off his own arm in the true story movie.
The Danny Boyle directed film is based on the real life story of rock climber Aron Ralston who had to amputate his own arm when it became trapped by a boulder in Utah.
He was trapped for five days with no-one to save him when he made the decision to sever his own arm and try to climb to safety.
Ralston wrote a best-selling book Between a Rock and a Hard Place about the experience, which the new movie by Slumdog Millionaire director Boyle is based upon.
But the pivotal scene caused a dramatic reaction at its first screening in Toronto, with critics describing it incredibly realistic and stomach turning.
"I cannot remember a reaction to a film like this in a very long time, perhaps not since The Exorcist sent audiences scurring for the doors," critic John Foote wrote.
"At the screening Monday there were several mad dashes to the door when these scenes came on, and more than a few moviegoers were looking anywhere but the screen."
Despite sending some running for cover, the movie received a standing ovation at the screening.
The flick - not for the faint at heart - hits screens in the US shortly and is scheduled for a March 17, 2011 release date here.
